$50 Application fee for all students.
Statement of Support form – Must be notarized by a notary public.
Bank Certification - verification on bank letterhead signed by a bank official showing at least $22,000 USD available to the student or equivalent.)
Tuberculosis (TB) exam results
Transcripts (proof of high school graduation and/or Transcripts)
Proof of English Proficiency - TOEFL Score must be at least 450+ on written, 133+ on computerized test, or 45+ on Internet Based Test. We also accept equivalent test such as the IELTS with the score of 4.5-5.5 and iTEP with the score of 4. Not required for students currently in the U.S. and Intensive English Program (IEP) Students.

Copy of Passport, Visa, and I-94
Copy of ALL I-20’s (SEVIS and Non-SEVIS I-20’s)
Transfer Authorization Form - Only necessary if transferring from another U.S. college or institution; Must include SEVIS release date.
Mandatory Medical Insurance Coverage - Students may pay for international Student Insurance online or at the Office of International StudentServices.
Dependents: Put the name, date of birth, country of birth, relationship to the F-1 student for each dependent (spouse and children) on a separatesheet of paper. All dependents must have medicalinsurance. Add $2,500.00 to the Bank Statement requirement for each dependent.
SEVIS fee of $200.00 - The quickest way to process the SEVIS fee payment is online at www.fmjfee.com Make sure to print out your confirmation ofpayment. You will need it for your Visa interview.
