Supervised Tutoring Hours Report: complete one form for each contract 

Priority Registration Contract: complete this form each semester (Fall and Spring only)

Academic Progress Report: complete this form each semester (Fall and Spring only)

Student Handbook: all SHP students are required to read and follow the rules/guidelines listed in the handbook

IGETC (Plan C) Worksheet: SHP students are required to meet with one of our SHP counselors (Marvelina Barcelo, Brittany Lundeen, and Rene Lozano) on a regular basis.  An Education Plan is required of all students.  Keep our counselors informed of any changes to your transfer plan.  If you do not meet with our counselors, you will lose all of the transfer benefits of the honors program.TRANSFER GUIDE (very informative)

Intent to Transfer Form: complete this form at the beginning of your last semester at Cerritos College

Graduation Biography Form: complete this form four weeks before the SHP Graduation Ceremony

Scholars' Honors Club Constitution: the current constitution for the SHC

UCLA Library Card Form: complete this form at least a month before you plan to use the library at UCLA

UCI Library Card Form: complete this form at least a month before you plan to use the library at UCI
