Criteria For Proposed Faculty Agreement Form

Directions for Completing Proposed Staff Development Activities for Faculty Agreement Form

  1. Review the offerings contained in the Staff Development Schedule.
  2. Select activities that meet your needs and complete the Proposed Staff Development Activities for ‘Faculty Agreement form. Keep a copy for your files.
  3. Note the number of Flex hours attached to each activity.
  4. Remember that the total flex hours required is 25.
  5. Send the Agreement to your Division Dean before October 1.
  6. If a workshop or training session is closed, you will be notified so that you can revise your form.
  7. Please notify Debbie Thomas, in the Staff Development Office, extension 2796, with any changes to your agreement during the school year
  8. Evaluations will be used for each activity and your thoughtful completion of these online surveys is most appreciated so that we can better serve the college in the future.
  9. If you are planning an Individual Staff Development Project, it must be related to teaching and/or learning.
  10.  Attach the Individual Project Agreement form to the Activities Agreement form.
  11. If you are planning a Departmental Staff Development Project, remember that it must be related to teaching and /or learning. Attach the Departmental Staff Development Project Agreement form to the Activities Agreement form.

Please keep in mind that popular workshops and training sessions fill early. So, the sooner that you turn in your form the better chance you will have of getting your first selections. A phone call to reserve a seat is the very best insurance of reserving a seat. Call extension 2796 for any questions or help in filling out your forms.
