To make a donation to program, please complete the donation application below.  Your donation will be evaluated by our staff to determine whether the department can use the donation for the program. A staff member will follow up with you by phone to coordinate a delivery or pick up within 3 business days. 

To receive an acknowledgement letter for tax purposes, you will be required to sign and date this form upon delivery or pick-up .  Should you have any questions or need assistance during this process please contact us at (562) 860-2451 x2986.
